Include these forms in your intake:
1. Systems Assessment Form or the original Systems Survey form (to be filled out by patient)
2. History of meds/surgeries (to be filled out by patient)
NOTE: Thorough completion of all forms will allow us to investigate the underlying root causes to any health challenge and allow us to create a designed clinical nutrition program specific to each person.
3. Symptom Tracking Sheet (Landscape) or (Portrait) (to be filled out by practitioner during initial consult)
